Position Summary
- Navigate for a cancer outreach program is a specialized role focused on connecting underserved or hard-to-reach populations with healthcare services through deliberate community engagement.
- The Patient Advocate acts as an intermediary between the community and healthcare systems, identifying individuals who may lack access to care due to barriers such as poverty, absence of insurance, geographic isolation, or distrust in medical institutions.
- The role involves actively interacting with these groups-usually through events, house visits, or partnerships with local enterprises-to educate them about available resources, evaluate health needs, and facilitate access to care.
The primary responsibilities include:
- Assessing individual needs, providing culturally appropriate health education, assisting with program enrollment, and coordinating follow-up care, including appointments or referrals.
- The Patient Advocate may advocate for patients by addressing social determinants of health, such providing transportation or linking them to food assistance programs.
- This position requires strong interpersonal skills, adaptability, and the ability to build trust across diverse groups. Expertise in the language, culture, and concerns of the target audience is often essential, as is collaboration with outreach teams, healthcare practitioners, and community leaders to improve the programs efficacy.
- The primary purpose is to reduce health disparities by ensuring equitable access to care and support.

- Bachelor of Science or Bachelor of Arts with experience in community oriented healthcare programs
Salary Range $15,174.83
